Conduct a pre-procedure verification process

The purpose is to make sure that all relevant documents and related information or equipment are:

  • Available prior to the start of the procedure

  • Correctly identified, labeled, and matched to the [patient]’s identifiers.

Elements of Performance

The Time Out is conducted prior to starting the procedure and, ideally, prior to the introduction of the anesthesia process (including general/regional anesthesia, local anesthesia, and spinal anesthesia), unless contraindicated.

 

The Time Out has the following characteristics:

  • It is standardized (as defined by the hospital). 

  • It is initiated by a designated member of the team.

  • It involves the immediate members of the procedure team including the proceduralist(s), the anesthesia providers, the circulating nurse, the operating room technician, and other active participants as appropriate for the procedure, who will be participating in the procedure at its inception.

  • It involves interactive verbal communication between all team members, and any team member is able to express concerns about the procedure verification.

  • It includes a defined process for reconciling differences in responses.

 

 

Includes Active Time Out for standardized and consistent Time Out performances.

 

Active TIme Out:

  • Always displays the hospital’s standardized checklist automatically when the patient enters the room

    LiveData electronic checklists are always up-to-date; they are easy to modify and refine as well as tailor to surgical specialties.

  • Provides one focal point for the checklist, displaying name of designated leader

  • Remote interactive "clicker" ensures participation of entire OR staff in close proximity to one another for easy verbal exchange

  • Displays one checklist item at a time, ensuring team focus on each item to be reviewed

  • Displays all relevant information beside checklist item

  • Displays names and roles of everyone in the OR, promoting active communication among team members

  • Automatically displays an individual checklist for each procedure

  • Includes a checklist item for reconciling differences, making it part of the standardized process

Facilitates handing off information as staff enter and leave the OR:

  • Key facts about the patient and operation are always visible

  • Real time information from physiological monitors and devices is always current

  • A continually updated list of team members in the OR is always on display, eliminating confusion when someone leaves the room

When able to address one another and view the same information, in the same format, at the same time, OR clinicians are in a good position to raise questions and red flags. Ultimately, OR teams are better poised as a unit to make timely, good decisions.


Helps standardize hand offs with other departments, such as the PACU and ICU:

  • Accurate, timely, comprehensive case reports can be generated and printed at any time

Automatically displays a snapshot of the patient’s medical history—including allergies—throughout the perioperative process. Allergic reactions and adverse drug events are, therefore, less likely to occur in the OR.
