Patient Safety: The Joint Commission (formerly JCAHO)

As shown below, LiveData OR-Dashboard helps hospitals meet the National Patient Safety Goals, a prerequisite for obtaining accreditation from the Joint Commission.

Through reinforcing Joint Commission protocols LiveData OR-Dashboard also provides a foundation for meeting quality of service payments.

Patient Safety Goal

LiveData OR-Dashboard Aid

Goal 1

Improve the accuracy of patient identification.

1b. Prior to the start of any invasive procedure, conduct a final verification process.

A Timeout panel is displayed just prior to the onset of surgery.

The Timeout panel includes information and real-time prompts for assisting the OR team with their specific procedure.

Remains on display until safety pause  is officially completed according to hospital protocol.

Goal 2

Improve the effectiveness of communication among caregivers.

2c: Measure, assess and, if appropriate, take action to improve the timeliness of reporting.

2e: Implement a standardized approach to "hand off" communications, including an opportunity to ask and respond to questions.

Facilitates handing off information as staff enter and leave the OR. Key facts about the patient and operation are always visible. Real time information from physiological monitors and devices is always current.

Accurate, timely, comprehensive, and easy to read case reports can be generated and printed at any time.

Offers a standardized approach for carrying out major hand offs with other hospital departments, such as PACU and ICU.

A continually updated list of team members in the OR is always on display. Knowing who’s in the OR and being able to ask for help by name promotes good communication and eliminates confusion when someone leaves the room.

OR clinicians are in a good position to raise questions and red flags when able to speak easily with one another by name and view the same information, in the same format, at the same time. Everyone, not just the anesthesiologist, sees physiological trends, for example. Ultimately, OR teams are better poised as a unit to make timely, good decisions.

Goal 3

Improve the safety of using medications.

Automatically displays a snapshot of the patient’s medical history―including allergies―throughout the perioperative process. Allergic reactions and adverse drug events are, therefore, less likely to occur in the OR.

Goal 4

Eliminate wrong-site, wrong-patient, wrong-procedure surgery.

This goal requires that "prior to the start of any invasive procedure, conduct a final verification process to confirm the correct patient, procedure, site, and availability of appropriate documents.

A time out window with information specific to hospital protocol is automatically displayed just prior to surgery.

Patient case data—identity, surgical procedure, and other pertinent information— are displayed in the OR as soon as the patient enters the perioperative process. Clinicians can refer to it during case setup.

Goal 8

Accurately and completely reconcile medications across the continuum of care.

LiveData OR-Dashboard makes allergies visible to everyone in the OR.
