By visually communicating what has been done, what needs to be done, and who is doing what, we can help staff maintain focus on all pertinent details related to the surgical patient and facilitate process-related tasks. This is the premise behind all LiveData solutions. Since “seeing is believing,” we are continually reviewing data from our installations at various hospitals, as are the hospitals themselves, to validate the effectiveness of OR-Dashboard. Results from our most recent review confirm that communicating information in a highly visual way is effective and appreciated by staff. After more than 1,100 procedures and 8 months of usage, OR-Dashboard helped a major mid-western hospital significantly improve compliance with Surgical Care Improvement Project (SCIP) documentation guidelines. The guidelines require recording such items as administration of prophylactic antibiotic, hair removal method, and VTE ordered/given. During the assessment period, documentation performance rose from an average of 74% of the cases during the first 2 months to 89% of the cases over the last 2 months. This improvement was statistically significant (P=0.0006), and on a practical level, significant in helping the medical institution meet SCIP guidelines and their patient safety objectives. The data, collected during the period of February–September 2009 in 6 cardiovascular and thoracic operating rooms with OR-Dashboard, also indicated that the improvement was continuous. Correlating documentation performance over time resulted in a significant (P=0.05) linear relation (r=0.502), suggesting the acquisition of a new and durable documentation habit that is likely to persist beyond the assessment period. The observed improvements in SCIP documentation were attributed to the following OR-Dashboard features: Context-sensitive reminders: large colorful notification messages displayed at the center of OR-Dashboard flag the OR team if prophylactic antibiotic administration is not indicated by the time of first incision and remain on display until documentation occurs. Case Timeline information: the status of prophylactic antibiotic administration is displayed on OR-Dashboard, providing OR staff with time-sensitive information while they are in a position to act on it. Real-time performance monitoring and feedback: the status of prophylactic antibiotic administration is displayed on OR-Dashboard; as well as communicated to the patient safety manager, who can subsequently direct follow-up questions and any needed assistance to those clinicians who have relatively low compliance rates.
Although completion of documentation rather than that of its related tasks was the subject of review, it is reasonable to conclude that the two may be linked in the clinical setting. In other words, if prophylactic antibiotic administration was not recorded, there is a possibility that the antibiotic was not administered. Thus the documentation data can be used to identify potential impediments to reducing post surgical complication rates and improving outcomes. *The full report of this assessment study will be available at the end of January 2010. | Conference Update IHE North American Connectathon LiveData will again participate in healthcare IT’s only large-scale interoperability testing event, which also serves as a prerequisite for inclusion in the HIMSS10 Interoperability Showcase. LiveData will confirm interoperability of our solutions, including the new perioperative workflow management system, with incoming data from physiological devices such as vital signs monitors, infusion pumps, and ventilators in the Patient Care Domain. LiveData will also test the recently developed profile interactions for communication with Voice Over IP (VOIP) phones and other wireless communication devices. Attendees can also learn about LiveData OR-Dashboard and the Patient-Aware OR at the KARL STORZ booth. | LiveData Milestones New Installations OR-Dashboard went live in 4 operating rooms at Henry Ford West Bloomfield Hospital as part of the KARL STORZ Patient-Aware OR on October 2, 2009. The implementation included Active Time Out with the World Health Organization’s Surgical Safety Checklist. The broad rollout to additional ORs will continue over the upcoming months, along with studies currently underway to assess the effectiveness of OR-Dashboard. NewYork-Presbyterian Hospital will install OR-Dashboard in 4 new operating rooms at Weill Cornell Medical Center’s Advanced Therapeutic Services Center (ATSC). The ATSC installations will include Active Time Out for Time Out and closing checklists, as well as real-time tools for monitoring and facilitating activities related to SCIP. Advances in LiveData Solutions LiveData continues to work with several hospitals to outline a comprehensive perioperative workflow management system. Building upon OR-Dashboard and its underlying RTI technology, this latest solution extends LiveData dashboards, control point applications, and notification and reporting systems beyond the OR, to encompass all perioperative departments, systems, and devices into one integrated system. Designed to efficiently manage the entire perioperative workflow, the system will also provide provide a foundation for enabling hospitals to develop and refine practices that improve patient care and staff satisfaction. |
